Novo-Passit (Ново-Пассит) is a tradename that contains several active principles in its composition:

Ethanol
Level 2
High risk. Poorly safe. Avoid or use a safer alternative.

While breastfeeding it is recommended to avoid use of alcohol or do it occasionally and moderately. Drinking beer does not increase milk production.
Long term consumption may cause: sedation, failure to thrive, psychomotor delay in the infant. Also, one case of Pseudo-Cushing Syndrome was reported that disappeared after alcohol stop by the mother.
Despite of low increase of Prolactin serum level, acute consumption affects the infant with sedation, and, inhibits Oxytocin secretion in the mother and hence affects the ejection reflex of breast milk with a decrease of 10% to 25% of milk production.
Excessive acute ingestion by the mother may induce coma and seizures in the infant.
Both beer 0.0% and non-alcoholic (<1%) may be drunk by the nursing mother. Increase of anti-oxidant properties of the milk have been recognized.
Elapsing time required to resume breastfeeding after occasional consumption of alcohol by the mother in order to ensure disappearance of blood depends on mother's body weight (lesser weight needs longer time to wait), and, on the amount ingested (higher amount needs longer time). Stop breastfeeding for longer than two and a half hour for every 10 - 12 g of ingested alcohol: 330 mL of 4.5% beer, 120 mL (1 glass) of 12% wine, or, one cup of 40% - 50% liquor. Suggested time-periods to stop breastfeeding for a 60 kg of body weight woman are: 1 glass of wine = 2.5 hours; 660 mL of beer = 5 hours; 3 cups of liquor = 7.5 hours (See reference values at Can Fam Physician 2002)
Avoiding bed-sharing is recommended to mothers who have drunk alcohol.
When topically used as disinfectant alcohol is compatible with breastfeeding.
Valeriana officinalis
Level 1
Low risk. Moderately safe.
Mild risk possible. Follow up recommended.

At last update significant data on breastfeeding were not found.
A commonly used herb in many cultures and countries, even during pregnancy and breastfeeding with very few reported side-effects. Whenever not abused it has a low toxicity.
Moderate use is considered to be compatible with breastfeeding, however because of the possibility of sedative effect in infants should better be avoided in cases of prematurity and in the neonatal period. Be aware of sedative effects in the infant.
Roots, rhizomes and stolons of the plant are used. It contains iridoids, valepotriates, steroids, essential oils, GABA and tannins.
Unproven beneficial effects in adults: sedative, hypnotic, anti-spasmodic.
Indication after Commission E of German Ministry of Health: insomnia, nervousness, anxiety.
Coffee
Level 1
Low risk. Moderately safe.
Mild risk possible. Follow up recommended.
Comment
Infant intake after usual daily consumption of the mother is lower than usual recommended dose for neonatal apnea treatment.
Elimination-time period may last from few hours in adults, to 3-4 days in the newborn infant.
At higher dose (more than 300 mg per day) caffeine may induce irritability, tremor and insomnia in the infant. However, some infants may develop irritability at a lower dose; in those cases the mother should decide appropriate coffee intake. Some studies have failed to show harmful effect among infants whose mothers were strong coffee consumers even during pregnancy.
Daily intake as high as 1 liter or more has been associated to anemia and iron deficiency in mothers and breastfed infants. Also, has been related to the Raynaud's phenomenon in the nipple of nursing women.
Mean Caffeine content: 1 coffee cup: 100 mg, 1 black tea cup: 80 mg, 1 green tea cup: 50 mg, 1 liter of cola & soda and energizers beverages 100 to 340 mg.

Fosfomycin
Level 0
Very low risk.
Compatible. Not risky for breastfeeding or infant.
Comment
Excretion into breast milk is not significant and no side effects on breastfed infants of treated mothers have been shown.
Approved for use as treatment on infants younger than a one year old.
A low oral bioavailability would protect the infant, except in the neonatal period or cases of prematurity. Calcium contained in the milk prevents it from absorption by the gut.
Be aware of the possibility of false negative results of bacterial cultures on febrile infants when the mother is on antibiotics.
Sertraline
Level 0
Very low risk.
Compatible. Not risky for breastfeeding or infant.
Comment
Excreted in tiny amounts into breast milk. Serum levels of breastfed infants whose mothers are on Sertraline are usually undetectable or very low. No harm effect has been observed on health and short or long term development of infants.
Transient troubles in the early neonatal period like drug withdrawal syndrome among newborn or premature infants with high serum levels as a result of treatment with Sertraline to the mother during pregnancy have been observed.
Sertraline causes fewer problems related to galactorrhea than other antidepressant drugs.
It is probably the safest antidepressant medication while breastfeeding.
Mothers who are treated with antidepressant medicaction are in need of stronger support for a higher risk of early breastfeeding
failure.
Ibuprofen
Level 0
Very low risk.
Compatible. Not risky for breastfeeding or infant.
Comment
Either undetected or insignificantly detected in breast milk.
Commonly prescribed to children by pediatricians.
Preferred medication for pain relief in nursing mothers.
The American Academy of Pediatrics rates it compatible with breastfeeding.
